The Jefferson Rehabilitation and Health Center is an assisted living facility at 200 North Pine Hill Road in Ketona. It is operated by Jefferson County and the Jefferson Health System. The director is Derrick Williams.

The center was constructed in 1995 with short-term rehabilitation as its primary focus. The center features a rehabilitation gymnasium, five dining areas, eight visiting areas, four outdoor courtyards and a beauty/barber shop. In addition to rehabilitation services, the center offers long-term nursing care.

In 2010 the Jefferson County Commission recommended selling the center to a private operator. Uncertainty about the future of the facility prompted many employees to move. That and the county's hiring freeze have left the center understaffed.
