The 2003 mayoral election for the city of Birmingham was held on October 14, 2003 with 18 challengers vying for the office held by incumbent Bernard Kincaid. Kincaid and Carole Smitherman advanced to a runoff on November 4. Kincaid held onto his post with 54% of the votes cast.

A principal issue during the election was an ongoing standoff between the mayor and the Birmingham City Council. All candidates pledged to break the gridlock and work with the council. Kincaid's campaign benefitted in the runoff from a $150,000 from the Voter News Network headed by Donald Watkins. Kincaid had refused a $17,500 donation from the same group in the general election.
